Instructions for completing enrollment card:

  1. Go to the American College Student Association home page - www.acsa.com
  2. Select Insurance and Other Plans
  3. Select Health Insurance
  4. Choose State
  5. Choose School
  6. Choose a Plan

New Members (skip to step #12 if you are already a member)

  1. Click "Use our Online Application"
  2. Complete step one: "Your Premium"
  3. Sign-up for ACSA Membership
  4. Complete Personal Information & Sign-Up
  5. Go to step #17 - #20

Members

  1. Verify membership information
  2. Update personal information and return to steps #2 - #6
  3. Click "Use our Online Application"
  4. Complete step one: "Your Premium"
  5. Enter User Name & Password
  6. Display Form
     
  7. Print Enrollment Card
  8. Indicate coverage & payment plan
  9. Sign (signature required) & mail enrollment card to:
